7. Duration of Data Storage
We will retain your information until your child reaches the age of 25 years or seven years after their most recent visit, whichever is longer. After this period, we will destroy or permanently de-identify the information, provided it is no longer needed for clinical or administrative purposes.
8. Interpreter Services
To enhance patient care and safeguard confidential information, InformADHD Children's Clinic avoids using family members as interpreters when patients and/or their caregivers do not speak English. Instead, for non-English-speaking patients and caregivers, we coordinate with TIS National to provide phone interpreters, ensuring effective communication during appointments (https://www.tisnational.gov.au/). Please be aware that if the patient or caregiver in need of an interpreter does not possess a valid Medicare card, an extra fee for the interpreter service during clinic appointments may apply, as the TIS free phone interpreting service is applicable only to Medicare-rebateable services.
9. Access to Medical Records
Requests for access to medical records must be made in writing. We discuss all requests with the treating doctor and offer alternative options if access cannot be granted due to concerns about health or wellbeing.
When seeking access to another patient's information, we may require the individual's consent. This applies to accessing a family members health information as well. Acknowledging that children aged 15 and above can request confidentiality regarding their healthcare information, granting them the ability to control access, we manage such cases individually, with assessments conducted by the treating doctor.
10. Consent and Authorisation
In order to provide optimal healthcare for your child, werequire your consent when completing our intake form, which is a prerequisitefor attending our clinic. Without your consent, we are unable to book a first appointment. The form includes sections covering various aspects of patient privacy and consent, including clinic administration, billing, communication with other healthcare providers, compliance with regulatory requirements, and research purposes. Additionally, as part of our medical record and practice management software, we utilise Halaxy. To ensure your understanding and agreement with this software arrangement, we ask for your consent for Halaxy'suse. We also provide information about our clinic's fees and cancellation policy to ensure transparency and a smooth experience during your child's assessment and management at InformADHD Children's Clinic. 11. Contact Information and External Complaint Bodies
